O disco foi idealizado por Cervantes Sobrinho, que assina a produção musical do álbum com Thiago Marques Luiz. Como Cervantes conta no texto escrito para a edição em CD (fabricada com tiragem de 200 cópias direcionadas somente a amigos da artista), o título João do Vale – Muita gente desconhece manda recado para as instituições culturais que se recusaram a bancar a produção de disco dedicado ao cancioneiro de João do Vale, compositor que deu voz ao povo nordestino, propagando na obra tanto a malícia sensual da região como as agruras enfrentadas pela população dos sertões de um nordeste historicamente castigada pela escassez de água.
Entre sucessos como Na asa do vento (João do Vale e Luiz Vieira, 1956), O canto da ema (João do Vale, Ayres Viana e Alventino Cavalcanti, 1956) e Coroné Antonio Bento (João do Vale e Luiz Wanderley, 1970), Milena reaviva músicas menos abordadas da obra do compositor. São os casos de Uricuri (Segredo do sertanejo) (João do Vale e José Cândido, 1957) e de Passarinho (João do Vale e José Longuinho, 1979), tema raro, lançado em disco há 40 anos e quase nunca lembrado.

Compositor que viveu o auge criativo na década de 1950, João do Vale ganhou visibilidade adicional entre 1964 e 1965, quando integrou o elenco do Opinião, show teatralizado que projetou Maria Bethânia, intérprete que deu asas a Carcará. Cerca de 55 anos depois do Opinião, Milena avoa com propriedade pelo nordeste de João do Vale.
